Upon enrollment, you will be paired with a dedicated Care Manager
who will help you connect to supportive services designed to meet your goals and needs.

Our dedicated Care Managers work with each member to develop a personalized service plan based on their strengths and needs. They collaborate with families to address any care gaps and offer additional support through Targeted Case Management and Intensive Care Coordination.
Care Managers also assist members in identifying areas of interest to explore during their time at New Earth.

• Weekly or bi-weekly check-ins to monitor mental, emotional, and physical well-being
• Facilitating exploration of enrichment programs and opportunities
• Engaging families to address gaps in care and assess additional needs such as groceries or rental assistance

• Developing SMART(S) goals (Specific, Measurable, Attainable, Realistic, Time-Bound, and Strength-Based)
• Guiding members in navigating systems to obtain necessary documentation or resources

Our Clinical Therapists offer mental health and therapeutic services to individuals, families, and groups. They specialize in assessing, diagnosing, and treating various mental health disorders and emotional challenges using evidence-based interventions.
Through psychotherapy, they help clients understand their emotions, behaviors, and thought patterns to foster positive change and improve overall well-being.

• Conducting initial assessments and evaluations to determine mental health needs
• Monitoring and documenting client progress throughout treatment
• Educating clients and families about mental health, treatment options, and coping strategies

• Developing personalized, strength-based treatment plans
• Collaborating with psychiatrists, social workers, probation officers, and case managers for coordinated care
• Adhering to ethical and legal standards, including client confidentiality and boundaries

• Providing individual, group, or family therapy using modalities such as CBT, DBT, and psychodynamic therapy
• Offering crisis intervention and support during emergencies
